He is survived by: his wife Kathy Stokes Hayes of Birchwood; his children, Jacqueline "Jackie" Gourley (Daniel) and Cody Dale Hayes (Ashley) of Cleveland; his grandchildren, Gabriella Gourley and Caleb Gourley; his son Nicholas Johnson of Birchwood; and his siblings, Diana Woody of Cleveland, Myleathie Callis (Roger) of Farragut and Terry Hayes (Pam) of Cleveland. He is also survived by several nieces, nephews and cousins.
