"Jack" Lewis and Nell Lewis; his siblings, Vernon "Corky" Lewis and Helen Ramsey; his brothers-in-law, Ogle Ingram, Harold "Whitey" Ramsey and John Stillwell; and his nieces and nephews, Peggy Lewis, Patricia Walsh, Adam Stillwell, Rusty Swafford and Bob Ramsey.
He is survived by: his wife Charlene Swafford Lewis; his children, Tammy Lowe (Gary), Debbie Silvers (Gary) and Timothy "Slim" Lewis; his grandchildren, Amber Silvers Price, Lauren Silvers and Katie Lowe; his great grandchild Case Edward Price; his siblings, Jackie Ingram, Jimmy Lewis (JoAnn), Larry Lewis (Lena) and H. R. Lewis; and his siblings-in-law, Anne Marie Lewis, Sharion Stillwell, Frances Williams (Mark), Scott Swafford (Cynthia), Haskell Swafford (Cindy), Ted Swafford (Sarah), Nathan Swafford (Natalie), Morris Swafford (Deborah) and Jeff Swafford (Annette).
