He was predeceased by: his parents, Evie Lee Sides and Mansfield Whitten Cook; his former wife Alma Jean MacArthur; his wife Beverly Barnes; his daughter Becky Jean Terrell; and his brothers, Thomas Fletcher Cook, Ted Cook and Melvin Frazier.
He is survived by: his children, Kenneth T. Cook, Gail D. Nelson, Tina M. Cook and Brenda C. Merritt; his siblings, Glennville Lee Cook, Donald Field Cook, Ellis Eugene Cook, Fred Cook, Roger Dale Cook and Joyce Ann Cook; his sister-in-law Ollie Cook; his grandchildren, William Nelson, Richard Nelson, Clay Cook, Kelly Cook, Hillary Richardson, Michael Hinkle and Sherry Sigler; and his great grandchildren, Jessie, Calib, Nicolas, Lucas, Hannah, Cooper, Rhett, Charlotte and Makayla. He is also survived by many other family members and friends.
