She was predeceased by: her parents, Dr. John G. Samuels and Ola Mae Samuels (Johnson); her former husband William F. Wade, Jr.; and her husband Forrest W. Minor. She is survived by: her children, William F. Wade, III (Angela), Leigh Ann Wade Dunlap and John Samuels Wade; her grandchildren, Christin Lorenson (Justin), Lindsey Walden (Patrick), William (Ryan) and Rebecca Dunlap; her great grandchildren, Alaina Lorenson, Savannah Wade (Walden) and Cayden Walden; and her sister Shirley Samuels Hurtubise. She is also survived by nieces and nephews.
In lieu of flowers, the family requests donations be made to the Asbury Methodist Church, The Burn Unit at Regional One, or to St. Jude Children's Research Hospital.
