He was predeceased by: his father Joseph Chadwick, Jr.. He is survived by: his wife Kathy Jane Chadwick; his children, David Chadwick, Jr. and Chemane Chadwick; his grandchildren, Alexandria Byars, Drake Chadwick, Devin Chadwick and Reece Chadwick; his brother Denny Chadwick (Betty Jean); and his cousin Al Hatcher (Patty).

Memorials may be made to the Wounded Warrior Project.
