He was predeceased by: his father Thomas L. Phillips, Sr.; and his brother Thomas L.. He is survived by: his children, Crystal Ammons (David Hunter), Kayla Jones (Zach), Sierra Phillips (Timmy Vanatta) and Autumn Phillips; his granddaughter Blakley Kate Ammons; his mother Gladys Phillips; his sister Rhonda Phillips; and his son-in-law Ryan Ammons. He is also survived by several extended family members and friends.
