She was predeceased by: her parents, Willie McIntosh and Odessa McIntosh (Jones); her husband James Franklin Middlebrook; and her siblings, Cofer, Harry, Jept McIntosh, Roena Booker, Rita Hurst and Mary Lewis.
She is survived by: her children, Glenda (Stanley), Charles Middlebrook, Teresa Oglesby, Jamie Middlebrook, Anthony Middlebrook, Frankie Yowell (Gary), Terri Coleman (Mac), Angela Middlebrook (Robert) and Dr. Chris Middlebrook; and her siblings, Willie Sue Samuel and Otis McIntosh.
