He is survived by: his mother Rose Baker Cook Sorg (Daniel) of Adamsville, TN; his daughters, Amber Cook and Jennifer Sullivan; his grandchildren, Jace of Selmer, TN and Jenna Belle of Selmer, TN; and his siblings, Johnny Cook of New York, NY, David Cook of Adamsville, TN and Terrie Kaiwie of Hawaii. He is also survived by many extended family and friends.
