He was predeceased by: his child Sammy Jo "SJ" Burtnett; his grandparents, Vera Mae, Duane Burtnett, Chet and Helen McNelly; and his father-in-law Mark Van Liere.
He is survived by: his parents, Dennis Burtnett and Mary Burtnett (McNelly); his children, Amery and Dayton; his wife Jocelyn Burtnett; his brother Dave Burtnett (Jill); his mother-in-law Lori Van Liere; and his brother-in-law Marcus Van Liere. He is also survived by many Aunts, Uncles, Cousins.
In lieu of flowers, a Memorial has been established in Sam's honor to help with the cost of funeral arrangements and a fund for his children.
