He is survived by: his sons, Kevin and Tad; his wife Nancy (Grevlos-Forrette); his children, Mike and Theresa Jameson (Brian); his stepchildren, Julie Schulte (Dick), Jane Kuhn (Sam), Janet Rokusek (Joey) and Jackie Bietz (Don); his sister Arlene Haase; and his son-in-law Terry Schuldt.
