He is survived by: his wife Carolyn; his daughters, Rebecca (John Hochstein), Laura (David Finken) and Paulette (David Cary); his grandchildren, Ethan, Kailyn, Emma, Claire, Isaac and Cecilia; and his sister Audrey (Bob Landers).

In lieu of flowers, the family requests that memorials in Guy's honor be given to the Sanford School of Medicine Tuition Assistance, South Dakota Pheasants Forever, and Christ the King School Tuition Assistance.
