He was predeceased by: his parents, John Phillips and Melba Phillips (Chase); his sister Janet Phillips; his stepfather Melvin Dutch Spaethe; and his stepbrother James Spaethe.
He is survived by: his sister Lori Perry (Andrew) of Louisville, KY; and his stepsister Sharon Horton of Sun Valley, CA. He is also survived by several nieces and nephews.
