He was predeceased by: his parents, Kenneth Lewis and Vivian Lewis; his wife Sharon Kay; and his sisters, Mary Jo and Kathleen.
He is survived by: his children, Forrest II (Rebecca), Kent (Misti) and Holly (Doug); his siblings, Alice and Ken; and his grandchildren, Damien (Amy), Brock (Lindsey), Jacob, Tate (DeLaney), Tya (Sage), Trace, Thad and Kort. He is also survived by his 7 great-granddaughters.
