He is survived by: his sons, Gavin Powell, Carson, Jackson and Peyton Patch; his parents, Sharon Patch (Harlan Anderson) of Madison and Bill Patch of Madison; his brothers, Chad (Susan Poncelet) of Madison, Travis (Jordan Rollefson) of Yankton and Tyler (Charlotte) of Sioux Falls; his grandmother Beverly Langner of Madison; his significant other Allison Nevins of Sioux Falls; and his niece Macy Patch of Madison. He is also survived by many aunts, uncles and cousins.

Memorials will go to set up a college fund for Dustin's four sons.
