He was predeceased by: his parents, Theodore Koins and Hattie Mae Harley Hoffman. He is survived by: his wife Kay N. Hoffman; his sons, Shelton Lavern Hoffman, Jr., Alan Hoffman (Melinda), Ted Hoffman (Dawn), Donnie Hoffman and Ronnie Hoffman; and his grandchildren, Shelby Kyzer (Addison), Koins Hoffman, Amity Hoffman, Harley Kay Hoffman, Townsend Hoffman and Gracie Hoffman.
