He was predeceased by: his parent Terry Dallas Hazelett. He is survived by: his mother Sandra Phillips Hull (Mike) of Parkersburg, WV.; his wife Jonika Denise Dale Hazelett; and his daughters, Chloe Elizabeth Hazelett and Lucy Jane Hazelett.
In lieu of flowers, memorials may be made in Donald Hazelett's name to Westminster Catawba Christian School, 2650 India Hook Road, Rock Hill, SC 29732.
