He was predeceased by: his parents, Harshall Smith and Edna Parker Cook; his wife Elizabeth Turner Cook; and his siblings, John Cook, Harshall Cook, Ara Cook Mathis and Christine Cook Tate.
He is survived by: his daughters, Judy and Vickie. He is also survived by two grandchildren, six great grandchildren and one great-great grandchild.
