He was predeceased by: his parents, Grady Cook and Essie Mae Mishoe Cook; his daughter Dana Marie Cook; and his siblings, Carl Cook and Hazel Hughes.

He is survived by: his wife Martha Cook; his sons, Chip Cook and Matt Cook; his grandchildren, Andrew Cook and Caroline Cook; and his siblings, Margie Ricks, Brenda Roberts and Lanny Cook. He is also survived by other extended family members and many friends.
