He is survived by: his parents, William Holm and Carolyn O'Sullivan; his wife Pam Holm; his stepfather Boyce O'Sullivan; his children, Kayla Holm, Brook Lee, Chris Lee and Kimberly Lee; and his siblings, Samantha Casas (Joel), David Inman III, Jordan O'Sullivan, Derek O'Sullivan, Mendi Just and Dustin Holm. He is also survived by 13 grandchildren.
