He was predeceased by: his parents, Phyllis Martin Phillips and Roy Phillips; and his siblings, Betty Lou Ford and Michael Brook Phillips. He is survived by: his daughter Elizabeth Anne Campbell; and his grandsons, William Andrew Campbell and John Richard Campbell. He is also survived by 3 nephews and 1 niece, 4 great nieces and nephews, and numerous friends.
