He was predeceased by: his parents, Thomas Otis and Georgia Kathleen Puckett; and his brothers, Millard, Charles and James.
He is survived by: his wife Gloria Ann Patton Puckett; his children, Billie Bailey (Charlie), Dale Puckett (Claudia), Ricky Puckett (Stephanie), Mary Thompson (Artie) and Jason Puckett (Shelby); and his sisters, Janice Horne of Tazewell, Virginia Griffith and Patsy Griffith of Conover. He is also survived by 12 grandchildren, 19 great grandchildren, 1 great great grandchild.
