She was predeceased by: her grandsons, Zachary and Jeremy Polyak. She is survived by: her sons, Luke Wayne Polyak, Jr. (Cindy) of Batesburg and James Michael Polyak (Deborah) of Leesville; and her grandchildren, Cody Polyak (Lindsey), Grant Polyak (Sherri) and Haley Perry (Sam). She is also survived by eleven great-grandchildren.
