Spivak (Kathleen); his grandchildren, Kayla Marie Hayes, Britney Elizabeth Spivak, Shawn Kole Spivak, Ava Brooke Spivak, Luke Thomas Fetzer and Abigail Lynn Fetzer; his great grandchild Vada Elizabeth Green; his children, Faris Rd Greenville, Box and Topeka; and also, David A. Spivak (Paula) of Painesville Township, OH and Daniel J. Spivak of Madison, OH.
