He was predeceased by: his wife Phyllis Barnes Lively; his parents, Lloyd L. Lively and Olive Stoner Lively; and his sister Kathleen Forker of Tucson, AZ.. He is survived by: his children, Lloyd L Lively (Adrianne) and Elizabeth Ann Planz (Libby) of Birmingham, AL. He is also survived by four grandchildren and three great grandchildren.
