He was predeceased by: his parents, John Thomas "Tom" Phillips, Sr. and Laura Ethel Hilton Phillips; his wife Ruby Doris Adams Phillips; his son-in-law Jack Pettit, Jr.; and his siblings, Wilford Phillips, Dorothy Deese, Idolene Phillips, Willie Mae Bowers and Lorene Baker.
He is survived by: his daughters, Denise Phillips Pettit and Tammy Phillips Mothershed; his grandchildren, Jack Pettit, III, Nicole Pettit Gordon (David), Amber Mothershed, Taylor Mothershed, Krystle Mothershed Williams (Blake) and Dalton Mothershed; his great grandchildren, Cody Gordon, Braxton Gordon, Chase Gordon, Addisyn Carpenter, Dakoyta Moore and Camley Moore; his nephews, W. T. Phillips and Bruce Phillips; his close friend Bleeka Carnes; and his caregiver Dianne Stewart. He is also survived by several other nieces and nephews.
