He is survived by: his wife Angela Lynn Altman; his children, Gene David Cook, Jr. (Jennifer), Bryce Rogers, Heather Lawson (Jimmy), Fallon Smith (Derrick) and Brittany Allison (Wesley); his grandchildren, Kaitlyn, Breanna, Kyra, Dallas, J. J., Hunter, Ethan, Sophie, Peyton, Aiden, Nathan, Autumn, Austin, Ariana, Ryan, Madison and Bryson; and his siblings, Doug Cook, Sr. (Bonnie) and Linda Smith. He is also survived by 5 great grandchildren, many nieces and nephews.
