She was predeceased by: her mother Maggie Adams; her husband J. Lee Cook; and her siblings, Rudolph Schaffer, Lucy McAlister, Foy Schaffer, Shirley Reed, Barbara Pearson, Judy Cook, Rosa Rita Schaffer and H. D. Shaffer.

She is survived by: her children, Ruth Anne Crews (Gene) of Cummings, Donald Lee Cook (Chrissy) of Cummings and Floyd Allen Cook (Karen) of Cummings; her grandchildren, Jamie Crews (Shelley), Chasity Nix (Travis), Bridgett McAlhaney (Jeffery), Hunter Cook, Faith Williams (Dustin), Allen Cook, Jr. (Courtney) and Brittany Cook Wells; and her siblings, David Schaffer (Ruth Ann) of Varnville, DiAnne Cook (Roy) of Varnville and Midge Mesanko of Punxsutawney, Pennsylvania.
