He was predeceased by: his parents, Jesse Everett Middlebrooks and Margaret Delores "Loky" Middlebrooks. He is survived by: his wife Leanna Middlebrooks; his brother John Middlebrooks (Misty); his children, Tara, Jack, Drew Middlebrooks (Emma Ingram) and Meg Thompson (Tavin); his granddaughter Merritt; his son Matt Middlebrooks; and his stepchildren, Collin Leach (Wendy), Autumn Laviolette, Olivia Leach, Kyla Laviolette, Camo Laviolette and Kenzley Laviolette.
