She was predeceased by: her parents, Stella Prentki (Tojek) and Adam Anthony Prentki; her husband Jerome Frank Kozacki; and her brother Adam Prentki (late Susan). She is survived by: her children, J. Scott Kozacki (Krista Kozacki) of Florence and Cheryl Frey (Bob) of Palm Bay, FL; her grandchildren, Stefan Kozacki, Jacob Kozacki, Emily Kozacki, Marissa Frey, Alexandra "Ally" Frey and Nicholas Frey; her great grandchild Jaxon Cabot; and her siblings, Henry Prentki (Susan) and Theresa Evans (late Richard). She is also survived by many loving nieces and nephews.
