He is survived by: his wife Kathy Mathewson; his children, Rick Cooke (Kelly), Greg Cooke (Shelley) and Barb Pellittieri (Craig); his grandchildren, Matt, Meagan, Sean (Kaitlin), Taylor, Marlee, Alexa and Bella; and his brothers, Dave (Sue), Dean (Anita) and Chris (Jenny). He is also survived by many extended family members and friends.

Per his wishes, in lieu of flowers, the family encourages memorial donations in Dan's honor be made to: CHC Learning Center, 1085 Eggert Road, Amherst, NY 14226.
