He was predeceased by: his parents, George Edwin Polk and Gladys Taylor Polk Brock. He is survived by: his children, Chad Polk of Red Springs, NC and Gina Webster of Laurinburg, NC; and his brothers, Shane Polk of Chester, SC and Curtis Polk of Myrtle Beach, SC. He is also survived by four grandchildren.
