He is survived by: his parents, Arnold Forman and Maria Munoz Forman; his sons, Nicholas Forman (Khadijah) and Vincent Forman (Mimi); his sisters, Joyce Romano (Mitch), Cynthia Dorizas (Dennis) and Lillian Forman; his brother-in-law Danny Peterson; and his grandson Vincenzo Forman.
