He was predeceased by: his parents, Joseph William Mazurak and Agnes Samenlowich Mazurak. He is survived by: his wife Evelyn "Evie" Mazurak; his children, Richard J. Mazurak, Jr. and Lora Mazurak; his brother Joseph W. Mazurak; and his grandchildren, Cyrissia, Benjamin, Shayleen and Kristen. He is also survived by six great-grandchildren.
