He was predeceased by: his wife Nell Powell Cook; his parents, John B. Cook, Jr. and Lucile Jackson Cook; his siblings, Doris C. Anderson and Dr. John B. Cook III; and his nephews, Brett A. McKee and Erik Anderson.
He is survived by: his sister Carolyn C. McKee. He is also survived by several nieces and nephews.
