He was predeceased by: his wife Lisa Vance; and his parents, Ernest H. Lewis II and Hazel Lewis. He is survived by: his daughter Hazel Lewis; his close friend Donna Lentz; his sister Beverly Jean Lewis; and his pet Archie.
