Lizak of Warwick, Rhode Island, born in Providence, Rhode Island, who passed away on April 11, 2024, at the age of 77, leaving to mourn family and friends. He was predeceased by: his parents, Frederick Lizak and Stella Lizak (Kurowski); and his siblings, Walter and Stella Belonos. He is survived by: his wife Ann Marie Lizak (Ryan). He is also survived by several nieces and nephews.
