He is survived by: his wife Elizabeth Kozak (Betty); his children, Brenda Gorman (Michael Gorman), Loree Silvia (Dana Silvia) and Tom Kozak (Rhonda Kozak); and his siblings, Tom, Richard, Robert, Leonard, Joann Rotola and Patty Waters. He is also survived by 6 grandchildren, 13 great-grandchildren, and 1 great-great grandchild.
