He is survived by: his parent Helen Cook (Brooks); his children, Lianne Cook of Gloucester, MA, Susan Cook Beaudoin of South Kingstown, RI, David Cook of Peabody, MA, James Cook of Blackstone, MA and Beth Cook of Fall River; and his grandchildren, Nicholas, Brian, Olivia, Gwen, Jamie Elizabeth, Ethan, Abigail, Leah and Nate.
