He was predeceased by: his parents, Herman Costa and Dolores Lewis (Paiva); and his brothers, Richard and Steven Costa. He is survived by: his sisters, Gail Costa, Lynn Carney, Donna Francis and Kathy Costa. He is also survived by many nieces and nephews.
