He was predeceased by: his parents, William H. Ballard-Smith and Agnes V. Ballard-Smith. He is survived by: his children, Gene A. Ballard, Jr. of East Providence, Agnes R. Ballard of Barrington and Kimberly A. Iannuccillo (Gregory) of Lakewood Ranch, Florida; and his brother Wayne D. Ballard (Barbara) of Stanardsville, Virgnina. He is also survived by seven grandchildren; three great-grandchildren and eleven nieces and nephews.
