He is survived by: his wife Christine Chiacu-Forsythe; his children, Jeremy Forsythe, Jonathan Forsythe (Mimi) and Kendra Forsythe Slafta (Andrew); his sisters-in-law, Debra Chiacu and Karen Recco (Chiacu); his nephews, Richard Overholt and Edward Overholt; his niece Kristen Ireland (Overholt); his great-nieces and great-nephews, Reece Overholt, Lauren Ireland and Matthew Ireland; and his cousin Deanna Henry (Aldrich).
In 2011, the Parnelli Awards recognized him with the Audio Innovator Award for his lifetime contributions to the industry.
