It is with great sadness that we announce the death of Carol A LaFazia of North Scituate, Rhode Island, born in Providence, Rhode Island, who passed away on May 5, 2024, at the age of 82, leaving to mourn family and friends. She was predeceased by: her husband Ronald LaFazia; her parents, Thomas Mazzotta and Carmela Mazzotta (Ciullo); and her brother Domenic Mazzotta. She is survived by: her children, Michael LaFazia of North Scituate and Lori Smith (Fred Gagnon) of West Greenwich.
Carol loved dogs and we are asking that in lieu of flowers please make a donation in her name to the Humane Society of the United States.
