He was predeceased by: his parents, Everett E. Cook and Margaret C. Cook. He is survived by: his wife Anne; his daughter Casey; and his siblings, Jeanne Elson Gordon, Judy Finnegan (Richard) and Robert E. Cook (Mary Ann). He is also survived by many nieces, nephews, friends, and extended family.
