She was predeceased by: her parents, David M. Moffat and Ellena Moffat (Hill); and her brother David Moffat. She is survived by: her children, Dawn M. Potter (Brian) of Hope Valley, Timothy J. Van Wormer, Jr. and Heather E. Beauchemin (Jason) of Narragansett; her siblings, Richard B. Moffat of Johnston, AnnMarie Carter of Johnston and Gail Masso of Putnam, CT; her grandchildren, John Moffat, Christian Clark, T. Johnathan DiBiasio, Nicole Sankey, Kelsie Van Wormer, Timothy J. Van Wormer, III, Corey Van Wormer, Stephen A. Beauchemin of Amelia, E. M and David Beauchemin; and her sister Sandra Kelly. She is also survived by four great-grandchildren.
