Polacek (Harrisville, Rhode Island), born in Providence, Rhode Island, who passed away on October 1, 2019, leaving to mourn family and friends. She was predeceased by: her parents, Louis Sbraccia and Giulietta Sbraccia (Sghelli); and her brother Louis Sbraccia Jr.. She is survived by: her husband John F. Polacek Sr. (Jack); her sons, John Polacek Jr. (Jennifer) of Harrisville and Michael Polacek (Rebecca) of Glendale; her granddaughters, Regan and Rayna Polacek; and her pet Sophie.
