He was predeceased by: his father Roger Fafard. He is survived by: his mother Gladys Fafard (Pol) of Blackstone, MA; his daughters, Melanie Gancarz (Scott) of Danielson, CT and Pamela Champagne; his sister Penny McMorrow (Michael) of Upton, MA; his grandchildren, Madison Rose and Gavin Wade Champagne; and his close friend Laurie Bartlett of North Smithfield.

In lieu of flowers, memorial contributions may be made to American Lung Association, National Direct Response, PO Box 756, Osceola, WI 54020-0756.
