She was predeceased by: her spouse J. Walter Choquette, Jr.; her parents, John J. Nelson and Mattie E. Nelson (Betts); and her son David E. Choquette. She is survived by: her children, Leslie P. Choquette (Paul Kreswick) of Worcester and Keith L. Choquette (Beth) of Bristol.
Due to Covid restrictions, a Memorial Service for Phyllis will be held at a later date.
