He was predeceased by: his parents, Roy Azulay and Dorothy Azulay (Duarte); and his brother Donald. He is survived by: his son Stephen Azulay, Jr.; and his siblings, Marilyn C. Pina of East Providence, William and Roy Azulay. He is also survived by many nieces and nephews.
