He was predeceased by: his wife Patricia J. "Pat" Costa (Faulkner); his parents, John Costa and Mary Costa (Amaral); and his children, Jeffrey John Costa and Linda Jeanne Costa.
He is survived by: his children, Steven Costa (Barbara) of Rumford and Annette Costa Geremia of Warwick; his grandchildren, Traci Williams, Derek Costa and Christy Silva; and his great grandchildren, Logan and Maddox.
