She was predeceased by: her stepparent Richard A. Cook; and her grandparent Joseph A. Lavallee. She is survived by: her parent Donna Cook (Lavallee); her grandmother Constance Lavallee of Cumberland; her godparents, Kevin Hackett, Debra Hackett of Cumberland, Uncle David and Aunt Cheryl Lavallee of East Providence; and her cousins, Kevin Hackett, Chris Hackett, David Lavallee and Desiree Lavallee.
In lieu of flowers, donations may be made in her memory to the ASPCA and the Jimmy Fund.
